CareTrack is a listed Greenway Marketplace partner, so there is no custom IT project. We connect through Greenway's certified partner APIs and pull your Medicare panel to identify candidates.

Enroll patients with consent in the chart
Your physicians approve the initial patient group. Our team handles outreach and enrollment, and consent is documented in the PrimeSuite chart from day one.

We run the monthly program inside PrimeSuite
Care calls, RPM readings, care-plan updates, and time logs are documented in PrimeSuite as we go, under your protocols and visible to your providers at any time.

Charges land in your claims module
At month end, billing-ready charges for completed CCM, RPM, PCM, APCM, and AWV services flow straight to the PrimeSuite claims module with no manual entry by your staff.

How do care-management charges get into PrimeSuite?

What does our staff have to do each month?
Very little. CareTrack's care team handles outreach, monitoring, documentation, and time tracking inside PrimeSuite, and charges post automatically. Your providers review escalations we surface under the protocols they defined.
We may move from PrimeSuite to Intergy. What happens to the program?
It carries over. CareTrack runs on both platforms through the same Greenway API family, so a PrimeSuite-to-Intergy migration keeps the same care team, the same enrolled patients, and the same workflow.
Which programs can we run on Greenway with CareTrack?
