CareTrack is a registered eClinicalWorks partner. Care-management documentation, monitoring data, and CCM/RPM charges land in the eClinicalWorks chart, so your staff never logs into a second system.
CareTrack connects to eClinicalWorks through eCW's certified FHIR interface, the standard interface eClinicalWorks offers to approved partners, and CareTrack is a registered eClinicalWorks partner with a published application. Each month our care team reads the clinical picture it needs from the chart (demographics, insurance, problems, medications, allergies, visits, lab results, immunizations, and vitals) and writes the results of the work back: the care plan and RPM summary as a clinical note, CCM and RPM charges on an encounter, and device readings for provider review. The record lives in eClinicalWorks, not in a separate portal.
The integration is the plumbing. The program is the people. CareTrack's care coordinators handle outreach, enrollment, monthly calls, monitoring, and documentation under your protocols, your providers review what we escalate, and your billers work charges from the encounter. Write-back to the chart is delivered through eClinicalWorks' partner write program, and we walk your practice through activation.

Running CCM and RPM inside eClinicalWorks

Activate the CareTrack app in eClinicalWorks
CareTrack is a registered eClinicalWorks partner with a published application, so there is no custom interface build. Your practice authorizes the app, we connect through eCW's certified FHIR interface, and we review your Medicare panel to identify candidates.

Enroll patients under your protocols
Your physicians approve the initial patient group. Our team handles outreach and enrollment, and each enrolled patient's care plan is filed to the eClinicalWorks chart as a clinical note from the first month.

We run the monthly program from the eCW record
Each month we read current problems, medications, allergies, visits, labs, and vitals from the chart, run care calls and home monitoring, and file the updated care plan and RPM summary back to the chart.

Charges land on an eClinicalWorks encounter
Completed CCM and RPM services are written to an eCW encounter with diagnosis codes, units, and rendering provider. Your billers work them from the encounter, with the supporting note in the same chart.

What does CareTrack read from and write to the eClinicalWorks chart?

Appointments are not part of the eClinicalWorks interface.
How do CCM and RPM charges get into eClinicalWorks?
CareTrack writes each completed service to an eClinicalWorks encounter with the diagnosis codes, units, and rendering provider. Your billers work the charge from that encounter, with the supporting note in the same chart. Write-back is delivered through eClinicalWorks' partner write program.
Where do RPM device readings show up in eClinicalWorks?
Device readings are filed to the patient's eClinicalWorks record for provider review, and staff can pull a reading into a progress note when it matters clinically. The monthly RPM summary note is the durable record of each month's monitoring.
What does our staff have to do each month?
Very little. CareTrack's care team handles outreach, monitoring, documentation, and time tracking, and files the results to the eClinicalWorks chart. Your providers review the escalations we surface under the protocols they defined, and your billers work charges from the encounter.
