What is the 24/7 Connect program?

It's a service your doctor's office offers through CareTrack that lets you reach your care team any time, day or night. If you have a question about your health, your medications, or just need help, you can call and a care coordinator who works with your doctor will help you.

Is this covered by Medicare?

Yes. These programs are Medicare-covered services offered through your doctor's office. Coverage details vary by plan, so before you enroll our team walks you through exactly how your plan covers the program, with no surprises.

Is remote monitoring covered by Medicare?

Yes. Remote patient monitoring and chronic care management are Medicare programs your doctor offers with CareTrack's help. You must have Medicare or a Medicare Advantage plan to be eligible.

Do I need to set up the device myself?

No setup needed. Your device arrives ready to use: no apps, no Wi-Fi, no passwords. Take it out of the box, press the button, and your reading goes to your care team automatically. If you have any trouble, call us and we'll walk you through it.

Who will be calling me?

A care coordinator who works with your doctor's office. It's the same team over time, so they get to know you, your medications, and your health goals. They share everything with your doctor, so nothing falls through the cracks.

Does this replace my doctor?

No. It connects you to your doctor between visits. Everything we do is shared with your doctor's office and recorded in your medical chart. Your doctor stays in charge of your care; we just make sure you're supported the other 361 days of the year.
