Out of the box, ready to go

Every CareTrack device arrives already set up. There are no apps to install, no Wi-Fi passwords, and nothing to charge overnight. Take it out of the box, use it, and your reading goes to your care team automatically.

Blood pressure monitor

Sit down, rest for a few minutes, wrap the cuff around your upper arm, and press the button. Your reading sends itself automatically.

Tip: Try to take readings around the same time each day. Your care team looks at the trend, not any single number.

Weight scale

Step on the scale in the morning, before breakfast if you can. That's it: the reading is sent to your care team on its own.

Tip: Daily weigh-ins help your care team notice fluid changes early, especially if you have a heart condition.

Pulse oximeter

Clip it on your fingertip and hold still for a few seconds until the numbers stop changing.

Tip: Warm hands give better readings. If your fingers are cold, rub them together first.

Glucometer

Use it the way your doctor showed you for checking blood sugar. Your readings are shared with your care team automatically.

Tip: Note how you feel when a reading is unusual; your care coordinator will ask, and it helps your doctor.

What happens to my readings?

A real person on your care team reviews your readings. If something looks off, they’ll call you (usually within minutes) to check in, and they’ll share anything important with your doctor. If a reading ever worries you, don’t wait for a call: (800) 835-1140. For an emergency, always call 911 first.
