Running CCM and RPM inside Epic

Identify eligible patients
CareTrack works with your organization to identify appropriate patients from the existing patient panel and align enrollment with program goals and physician approval workflows.

Enroll and connect patients
CareTrack supports patient outreach, enrollment, onboarding, and device setup. For RPM, patients receive connected monitoring devices designed to transmit readings from home without requiring another patient portal.

Monitor between visits
Incoming monitoring information is reviewed according to the organization’s established workflow so changes, trends, and readings requiring attention can be identified between office visits.

Follow up and escalate
CareTrack follows up with patients and routes information requiring clinical attention according to physician-defined escalation protocols. The physician and practice remain in control of clinical decision-making.
