Held on November 14, World Diabetes Day is an opportunity to raise awareness of diabetes and find solutions for better prevention, diagnosis and management of the condition.

Chronic Care Management (CCM) & Principal Care Management (PCM)

The CCM and PCM program provides patients with an individualized care plan geared towards managing the patient’s diabetes mellitus. Patients work with our Care Team to help control blood glucose levels, prevent complications or other conditions.

We consistently check in with patients for better adherence to treatment plans, medication management, education and more. 

Remote Patient Monitoring (RPM)

RPM provides patients access to a glucometer that notifies our care team of any changes. We are also available to walk patients through the process of monitoring their glucose levels to improve and reduce their symptoms and any escalation. 

These user-friendly RPM devices, along with our CCM and PCM, give patients the support they need to treat and improve their diabetes symptoms during World Diabetes Day and every day.
