The problem: blood pressure control they couldn't see
Cedar Point Health's quality scores for hypertension were coming in lower than expected, and office visits only produced one blood-pressure reading at a time. The group wanted real-time analysis of how patients were actually doing at home, starting with blood pressure, the number one cardiac risk factor in their population.
The program
CareTrack supplied the full RPM service: identifying the patient cohort, recruiting and enrolling patients, shipping FDA-approved devices with training, monitoring readings with a tailored triage structure, and integrating everything with the group's Greenway PrimeSuite EHR, including billing that flows straight to the claims module hands-free.
Enrollment began in August 2024 with hypertension as the initial use case. As clinical needs were identified, the program expanded beyond blood-pressure cuffs to weight scales, pulse oximeters, and glucometers, targeting cardiovascular expansion including stroke, ischemia, COPD, and diabetes-related risk.
The results
By spring 2025, more than 600 patients were enrolled. Patients with sustained engagement saw blood-pressure improvement, with more uncontrolled patients becoming controlled, measured by many home readings rather than a single office visit. CareTrack also handled the CMS requirement of 16 days of readings per 30 to bill the device code, keeping billing clean while the practice focused on care.
For an independent primary care group facing inflation and shrinking reimbursements, RPM became a positive-margin line of business without a larger staffing footprint.
